“Community Care Connect is on a mission to support Carer-led start-ups, cooperatives and other community sector organisations to develop and deliver their own Carer Introduction Services.
It promotes a localised approach to building care and support networks that enables communities themselves to play a vital role in meeting the care and support needs of the people that live there. Their vision is of communities themselves working to fill gaps in care and support, increasing choice and control for the people that draw on services.
It works by providing start-up and ongoing support for people developing new services, including tools and an online platform that enables people using services and those that work with them to find and meet each other, to work together within care teams, to organise their care and to manage payments and billing.
